If you are in a position to negotiate then do it. Salary negotiations shouldn’t depend on cost of living. It should depend on your skillset, your role, your current salary etc.

I would definitely suggest talking to dieticians at Fittr community(Fittr app),they have doctors as well. Also most of your problems are fixable with a proper diet and exercise routine. Would highly recommend strength training and following a balanced and nutritious diet.
