People mention attitude, but the answer is related to socio-economic development and lack of governmental regulations. India has developed significantly in the last 30 years with a boom in middle class families. Middle class families want and can achieve a clean home environment.

However, India is still a developing country with corruption/lack of government regulations. The average middle class Indian cannot do anything to fix their local dirty bakery. But I'm see more and more posh clean restaurants in India come up, especially in high income areas in Tier 1 cities. As India develops, so will its cleanliness.

You mentioned America in your other comment. Local governments in the US have "Departments of Public Health" that perform regular health safety inspections and shut down any place that does not meet those standards. But if you go to poorer places in America where people pay less tax (due to lower incomes), the local Department of Public Health doesn't have much funding to ensure food safety. You'll see a lot of dirty/dingy restaurants in poor areas of America too.
