Just lost a big account, what I am doing wrong?

I have a local SEO client that ranks over 150 high search volume keywords within the top 10 positions in a highly competitive industry. This marks the second client I've lost in the past 90 days, even after delivering excellent results. It's disheartening, but... Any tips on retaining clients would help
