How can a small startup compete against a well-established industry leader?

Hello everybody, I've had this question for a while now, are there any examples of small startups successfully overtaking or significantly challenging well-established competitors in any industry? What kinds of mistakes do large, dominant players tend to make that can create opportunities for smaller startups to take advantage of?
