Where does all the money go?

So, recently Adani donated about 10,000 crore rupees, and similarly, Tata used to donate 60-70% of his annual income. The Tata family has almost donated $100 billion till now. If Tata donates so much money, why do his medical colleges charge crores of rupees from students? Where does all the money go? Even if you want to use this much money for donations, you also need a good number of people to do the job. How much employment do these NGOs provide in the market?
