Portuguese Passport Help

Hey Guys, Does anyone knows how can i get the Alvará de Nacionalidade of my grandfather. We found his bilhete de identidade (Green Book) and also a pager which says bilhete de identidade with Cidadao National mentioned. Our agent is asking for Elfonso. Can anyone share some info on this. Thank you
