WPC RTR undertaking

Hi guys, this might be a dumb question but I wanted to know what exactly am I supposed to to write for the undertaking that needs to be submitted to WPC for the issuance of an RTR P? I need to surrender my old (expired) RTR permit to apply for a new permit. Along with this I need to submit a signed “undertaking”. Was wondering what exactly should be the body of this undertaking?
