How to apply for a name change in Bangalore Gazette?
1. Draft a Name Change Affidavit:
- Prepare an affidavit affirming your old and new names.
- Get it attested by an Advocate SEM, Magistrate, Notary Public, or Oaths Commissioner.
- Use a stamp paper of Rs. 20 for printing the affidavit.
2. Publish a Name Change Advertisement in Newspapers:
- Advertise the name change in a regional language newspaper and an English daily.
- Include your old and new names, address, affidavit date, and attester's name.
3. Publish a Notification in the Gazette:
- Collect necessary documents: ID proof (Aadhaar, passport, etc.), address proof (ration card, electricity bill, etc.), and a recent photograph.
- If changing name after marriage, include the marriage certificate.
- Submit the documents to the Government Central Press in Bangalore for gazette publication.
