How to apply for a disability certificate in Bangalore?
Step 1: Visit the official website of the UDID card (https://www.swavlambancard.gov.in/).
Step 2: Click on the 'Register' button to create an account on the UDID web portal.
Step 3: Fill in the required personal, disability, employment (if applicable), and identity details.
Step 4: Upload the necessary documents and submit the application.
Step 5: Receive the login credentials (enrolment number/UDID number) on your registered mobile number and email ID.
Step 6: Login using the enrolment number/UDID number and date of birth.
Step 7: Click on 'Apply online for Disability Certificate' and upload the required documents.
Step 8: The CMO Office/medical authority verifies the data and assigns specialists for assessment.
Step 9: The specialist doctor assesses the disability and provides their opinion.
Step 10: The medical board reviews the case and assigns a disability percentage.
Step 11: The CMO Office generates the disability certificate and UDID card, which will be dispatched to the disabled person.
