PM jay card in bangalore

Hi, I am new to bangalore and trying to get PM jay card for my maid and her son. She’s struggling with health care expenses and having the card would really help. Is this something that we can get at a e seva / aadhaar card Kendra near by? What other documents does she need for this? Thanks
