What's the procedure to get duplicate marksheets of 10th, 12th and Graduation (and degree)?

A cousin of mine has lost all his educational documents, wanted to know what's the procedure to get marksheets for 10th and 12th (Both done through MP Board) and Graduation semester marksheets and degree which was done under RGPV. If anyone has any idea, it would be a great help.
