Leave Without Pay during 5 year probation in Gujarat Government

Hello, I work as nurse in government hospital which is under Commissioner of Health, Gujarat. I have finished 3 years of probation (of total 5 years). Now, I need to take 3 months of leave without pay to visit my spouse who is living abroad. How to properly take this LWP without facing any problem in future when service will become regular after 5 or may be 5 years 3 months? Should I mention visiting abroad and take NOC while taking leave? Will my 3 month LWP create any problem in future while regularization of the service after probation?
